Sports Physical

If you are registering for a sport, a current physical must be on file with the Centennial High School Activities Office or you will be unable to proceed with registration. Physicals are considered current for 3 years and must not expire during the season for which the registration is being made. 

Families may check the status of their child's physical date in Campus Parent Portal - Navigate to More and click Athletic Eligibility. The most recent physical date is listed. If dates are not listed, a sports physical is not currently on file.

Completed physical forms may be submitted to the Athletics & Activities Department at Centennial High School.
