How often do I need a Sports Physical? 

Every 3 years new documentation is required to certify that each student has been medically evaluated and are deemed to be physically fit to participate in school activities.  

What if my physical expires mid-season, am I still OK to participate?  

No,  Actually if your Physical Exam will expire during the season, you will not be able to register for the activity.

Can I see if my child's sports physical is already on file with the school?

YES! Parents can check online through Infinite Campus/Parent Portal to view the health history of their student by going to More > Athletic Eligibility. If dates are not listed, a sports physical is not currently on file.

I submitted my sports physical form to the office. When will I be able to register?

We typically ask for a grace period of 24 hours between the time the physical form was submitted to the time you can log on and register. The office staff is extremely busy and at times may not be able to process data or input information into the system right away. If you are still unable to complete registration after waiting the 24-hour period, please contact the office at 763-792-5018. Please do not wait until the registration deadline to submit physical forms and expect to register that day.

Where do I turn in my Sports Physical Form?  

Please submit your physical to the Athletics & Activities Department at CHS West Office. The form can be scanned and emailed to CHSActivities@isd12.org or fax to 763-392-7274. 

Where should I take my child to get a sports physical examination?

The most common place to get a sports physical exam is the child's primary care physician or pediatrician's office. Any certified medical health professional can complete the physical exam and must sign and date the form.

There are select Walgreens stores and Minute Clinics that offer free sports physicals. You can search their websites to learn more information. Sport clinics like Summit Orthopedics also offer free sports physicals during the summer months and throughout the year. Pre-registration on their website is required.

What if I need a Physical very 'Last-Minute'?  

You don't have to wait to get in with your family clinic if the reservations are backed up.   You can walk into any Minute Clinic and the Dr onsite can complete a Sports Physical for you.

Can my child register for sports without seeing a doctor? Is there a waiver if I don't have a sports physical?

No, you cannot complete online registration without a valid sports physical on file. All student-athletes must be examined by a certified health professional who has cleared them and deemed them medically eligible to participate in all school interscholastic activities. This is a requirement from the Minnesota State High School League. There is no waiver or exemption.
